As the Owner/Representative for the above company, I certify the equipment listed above will be placed at the above premises, kept in good condition, and acknowledge this equipment and decor is the property of RESNICK DISTRIBUTORS. I also acknowledge the following provisions as set forth below: PRODUCT VOLUME: It is understood that this equipment and decor is loaned to the merchant based upon sales of products sold by Resnick Distributors. The merchant is required to maintain its current level of business from Resnick. MAINTENANCE: Daily cleaning and regular maintenance by the storeowner is required. PURCHASING OF PRODUCT: All products used in the equipment listed above must be purchased through Resnick Distributors. Any use of other products in the equipment will result in immediate termination of the program. REMOVAL/RETURN OF THE UNIT: It will be the responsibility of the owner/representative to ensure the return of the unit(s) in the same condition as it was received, normal wear and tear excepted. If the unit is not returned to Resnick Distributors, it is the responsibility of the store to reimburse Resnick Distributors for the value of the machine. CANCELLATION OF THE PROGRAM: Either party may cancel the program at any time with a 30 day written notice. If the customer cancels the program it will be their responsibility to return the equipment. Customer is also responsible for any expenses incurred with the return, including legal, shipping, and other related expenses. Additionally, Customer must inform Resnick Distributors if the store is sold or if there is a change in ownership. Resnick reserves the right to pick up the equipment if not informed of this change.
